- Coordinates the implementation of MSF clinical policies, tools, guidelines and protocols (antibiotic prophylaxis, post-operative pain management, thrombo-prophylaxis, caesarean section, blood transfusion, etc) and recommendations (standard hygiene and sterilization rules, prevention to HIV, TB, Syphilis and Hep B exposure) in order to minimize clinical risks.
- Undertake pre-operative consultations, evaluating the surgical risks with the anaesthetist and/or other medical staff involved, and obtaining the patient’s signed consent to operate.
- Carry out emergency and programmed surgical intervention following MSF protocols and policies including swab counts, ensuring appropriate use of surgical equipment and proper OT registry entry/documentation of all OT intervention.
- Carry out post-operative clinical care and ward rounds collaborating with the anesthetist, the Operating Theatre Nurse and ward staff or other Unit Managers to ensure compliance with MSF protocols and rational use of medicines. Attend OPD, emergency room when required.
- In collaboration with the anaesthetist, carry-out the collection and analysis of quantitative surgical- anaesthesia data, using post-operative observation sheets, routine data collected, patient files, operating department register, patient file, etc to monitor quality. Monthly revue data and analyze clinical trends and comment on adverse events / poor clinical outcome. Support organizing the operating program and with OT staff monitor use of material and equipment and advise on instrument needs and coordinates patient referral.,
- Put in place the emergency disaster response plan of the hospital along with the anesthetist and the medical doctor and /or the responsible nurse and run role hospital practical rehearsals, in order to ensure the continuity of the services under any circumstance
- Knows and helps implement the accidental blood exposure policy.
- Plan, evaluate, and supervise the training of the staff (emergency and the out-patient teams) in order to ensure the amount of knowledge required and to improve people diagnosing, triage, and early detection capabilities.
- Supports other medical teams/wards when needed (i.e. midwifes and obstetric teams in the maternity ward, etc.)
MSF Section/Context Specific Accountabilities
Patient care
§ Provide direct good quality medical and surgical plans and care to the patient in the emergency room, observation room, IPD, OPD, operation theatre and ICU.
o Conduct assessment of patients that requires surgical opinion in ER and collaborate with medical team about the treatment plan
o Initial assessment, stabilization and surgical management of severe and non severe burn injuries
o Regular rounds and follow-up of patient admitted in IPD and ICU that falls under their area of experties (e.g. General surgeons for general surgery cases)
o Medical consultation conducted in OPD patients during their follow-up.
§ Be assured that pre-op, intra-op and post-op surgical plans are followed properly.
§ Visit and provide support for patient care in the emergency department. (Eg- discussion about the patient for care plan/admission, etc)
§ Maintain clear documentation and up-to-date patient notes for all patients.
§ Discuss clinical cases with the supervisor/line manager and colleagues to improve the quality of care.
§ Strictly follow the MSF SOP, protocols, procedures, guidelines, and checklist to provide good quality of care.
o Ensure with OT Team that Instruments used during surgical intervention is properly counted and complete.
o Coordinate with different OT team members about the procedure and plans to ensure patient safety – Before, During and After the surgical interventions.
o Collaborate with different medical team about the treatment plans in ED, OT, IPD and OPD.
§ Inform the family and patients about the diagnosis, prognosis, medical treatments and decisions. Ask for consent when necessary.
§ Strictly adhere to IPC measures and participate in the promotion activities.
§ Maintain good and fluent communication with the medical staff (other doctors, surgeons, nurses, nurse assistants, mental health, anesthetists, physiotherapists, health promoters) and other staff (stretcher barriers, cleaners, crowd controllers) to improve the quality of care.
§ Maintain respectful behaviour with the patients, their families, and the MSF medical/non-medical team.
§ Ensure proper handover for the shift changes and patient transfer process (IPD, ER observation, ICU, etc)
§ Provide medical consultation of patients and collaborate with supervisor or OMS (and team) about the changes in management plan.
§ Actively participate in regular meetings and trainings
Documentation and Epidemiological data
§ Complete the patient information in the file and EMR to ensure proper documentation of the clinical findings, vital signs monitoring, diagnosis and treatment.
§ Proper registering patients (eg – OT schedule, OPD appointment schedule, Truelock, etc) and reporting (Eg – SSI, incident report, patient safety incident, etc)
§ Inform about difficulties regarding documentation to avoid missing information or the persistence of an error.
§ Present the daily patient data from the surgical department in morning medical meetings.
Training
§ Participate and organize training sessions based on MSF protocols.
§ Coaching and training the new MD national staff (Induction sessions)
Multidisciplinary team
§ Actively participate in the necessary meetings– monthly meetings, case studies, MMR, and mortality audits meetings, etc
§ Present, discuss and provide suggestions for the complicated cases from IPD, OPD, night/day shift, daily operation plan cases, and operated cases with the surgical team
§ Prepare and present, to the rest of medical team, mortality review and/or case review
§ Participate in amputation committee meetings and true lock case discussion meeting
§ Ensure multidisciplinary team discussion (including family meetings)and decision is taken for complicated/critical cases
§ Ensure good teamwork with other medical staff and observe the schedule of their activities including:
o Physiotherapy Department – discussion of cases and treatment plan.
o Involvement and collaborate with AMS doctor to ensure that ATB protocol is been implemented and followed.
§ Provide feedback and suggestions to supervisors for the improvement in patient care management
Infection Prevention and Control
§ Observe standard precautions and 3 Pillars of IPC “Hand Hygiene, Disinfection and Transmission Based precaution” accordingly.
§ Provide support to IPC and related activities, including active participation in implementing MSF IPC guidelines.
§ Address concerns related to IPC in collaboration with the supervisor/line manager and IPC supervisor
Others
§ Follow and support MSF medical strategy including strict implementation of Admission Criteria.
Minimum Requirements and Competencies
Education
- Medical Doctor diploma with specialization certificate in general surgery
- Desirable diploma in emergency medicine
Experience
- 2-year specialized surgical experience minimum and desirable working experience in trauma care.
Languages
- Local language is essential. (Pashto and Dari). The English language is essential.
